ABSTRACT

Objective: To investigate the clinical effects of one-stage revision combined with intra-articular infusion of vancomycin in the treatment of chronic prosthetic joint infection (PJI) caused by Enterococcal. Methods: From May 2013 to June 2020,the clinical data of 9 patients (2 males and 7 females) with chronic Enterococcal PJI treated with one-stage revision using intra-articular infusion of vancomycin at Department of Orthopaedics,First Affiliated Hospital of Xinjiang Medical University were retrospectively analyzed,including 8 hips and 1 knee.A total of 9 patients with age of (63.9±11.7)years (range:43 to 76 years) were included, and the body mass index was (23.6±4.3)kg/m2 (range:18 to 30 kg/m2).There were 6 cases with antibiotic history and 5 cases with sinus tract.The joint fluid,infected tissue around the prosthesis and ultrasonic shock fluid of the prosthesis were collected during operation for microbial culture identification and drug sensitivity test.After thorough debridement of the infected site and removal of the infected prosthesis,a new prosthesis was implanted,then the drainage tube in the operation area was placed.After surgery,vancomycin(1.0 g,q12 h) was combined with intra-articular vancomycin(0.5 g,qd) in monomicrobial PJI,and vancomycin(1.0 g,q12 h) was combined with intra-articular vancomycin (0.5 g,qd) and imipenem/meropenem (0.5 g,qd),and the interval between the two drugs was 12 hours in polymicrobial PJI.Hip and knee functions were evaluated by Harris Hip Score or Knee Society Score(KSS),respectively.The comparison of hip function scores before and after operation was performed by paired t-test. Results: All patients were followed up for (60±39)months(range:24 to 110 months).Two cases were infected with Enterococcus faecium and 7 cases were infected with Enterococcus faecalis.There were 7 cases of monomicrobial infection and 2 cases of polymicrobial infection.Erythromycin(5/9),tetracycline(4/9),ciprofloxacin and β-lactam antibiotics(3/9) were the top three antibiotics in Enterococci resistance rate.The sensitive antibiotics for Enterococcal were vancomycin,linezolid and tigecycline.The average duration of intravenous antibiotics was (14±1)days (range:13 to 17 days),and the average duration of antibiotics in articular cavity was (15±2)days(range:11 to 20 days).Mean duration of oral antibiotic use after discharge was (2±1)months(range:1 to 3 months).One case of polymicrobial PJI treatment failed,with a failure rate of 1/9.At last follow-up,the Harris score of patients with hip PJI increased from (43±6)points to (84±6)points(t=-11.899, P<0.01). KSS score of knee function was improved from 33 point pre-operatively to 85 point post-operatively;overall function score was improved from 35 point pre-operatively to 80 point post-operatively.During the treatment,no formation of sinus tract of the hip joint caused by a catheter,skin necrosis at the knee puncture site or leakage of joint fluid;no complications such as deep vein thrombosis and pulmonary embolism occurred. Conclusions: One-stage revision combined with intra-articular infusion of vancomycin can achieve acceptable infection control rate and joint function in patients with chronic Enterococcus PJI.However,the treatment of polymicrobial PJI still needs to be further verified.

ABSTRACT

ObjectiveThe purpose of this study was to evaluate the functional outcomes and satisfaction in periprosthetic knee infection patients after above-the-knee amputation (AKA). MethodsA review was performed in 13 patients who underwent AKA due to periprosthetic knee infection from October 2010 to August 2022, there were 7 females and 6 males with the median age of 69 (45~ 73) years. The mean number of surgical procedures between primary total knee arthroplasty and AKA was 2.9 times (range: 1 ~ 6). Patients were segregated into non-elderly group (including patients <65 years old) and elderly group (involving patients ≥65 years old). Functional outcomes were evaluated by SF-12 and Barthel activities daily living index (Barthel ADL index). Patient satisfaction was accomplished through a questionnaire. ResultsFour patients died within 6 months after the amputation. Three patients received surgical procedures for recurrent infections after AKA. In the remaining 9 patients,the median time from AKA to the final follow-up was 4.2 (1.9~8.2) years, three patients received surgical procedures for recurrent infections after AKA. Patients in non-elderly group showed better functional outcomes than elderly patients after AKA (both P<0.05). All patients in non-elderly group were fitted with a prosthesis. Whereas, elderly patients reported a better satisfaction after AKA than young patients. ConclusionYoung PJI patients seemed to have better function and ambulatory ability with high expectation and demand, which made them feel less satisfied about AKA. Meanwhile,despite poor function outcomes due to the less healthy physical condition, the alleviation of financial and psychological burdens resulting from reinfections and multiple surgeries and the less demand in function and daily activities made elderly patients have a good satisfaction after AKA.

ABSTRACT

Objective: To analyze the safety and efficacy of combined left atrial appendage (LAA) and patent foramen ovale (PFO) closure in adult atrial fibrillation (AF) patients complicating with PFO. Methods: This study is a retrospective and cross-sectional study. Seven patients with AF complicated with PFO diagnosed by transesophageal echocardiography (TEE) in Zhoupu Hospital Affiliated to Shanghai University of Medicine & Health Sciences from June 2017 to October 2020 were selected. Basic data such as age, gender and medical history were collected. The atrial septal defect or PFO occluder and LAA occluder were selected according to the size of PFO, the ostia width and depth of LAA. Four patients underwent left atrial appendage closure(LAAC) and PFO closure at the same time. PFO closure was performed during a one-stop procedure of cryoablation combined with LAAC in 2 patients. One patient underwent PFO closure at 10 weeks after one-stop procedure because of recurrent transient ischemic attack (TIA). All patients continued to take oral anticoagulants. TEE was repeated 8-12 weeks after intervention. In case of device related thrombus(DRT), TEE shall be rechecked 6 months after adjusting anticoagulant and antiplatelet drug treatment. Patients were follow-up at 1, 3, 6, 12, 24 months by telephone call, and the occurrence of cardio-cerebrovascular events was recorded. Results: Among the 7 patients with AF, 2 were male, aged (68.0±9.4) years, and 3 had a history of recurrent cerebral infarction and TIA. Average PFO diameter was (3.5±0.8)mm. Three patients were implanted with Watchman LAA occluder (30, 30, 33 mm) and atrial septal defect occluder (8, 9, 16 mm). 2 patients were implanted with LAmbre LAA occluder (34/38, 18/32 mm) and PFO occluder (PF1825, PF2525). 2 patients were implanted with LACbes LAA occluder (24, 28 mm) and PFO occluder (PF2525, PF1825) respectively. The patients were followed up for 12 (11, 24) months after operation. TEE reexamination showed that the position of LAA occluder and atrial septal defect occluder or PFO occluder was normal in all patients. DRT was detected in 1 patient, and anticoagulant therapy was adjusted in this patient. 6 months later, TEE showed that DRT disappeared. No cardiovascular and cerebrovascular events occurred in all patients with AF during follow-up. Conclusions: In AF patients complicated with PFO, LAAC combined with PFO closure may have good safety and effectiveness.

ABSTRACT

The proximal medial column of the humerus is a continuous cortical region in the inner and lower part of the humerus head, which has attracted more and more attention in clinical and scientific research since it was proposed. It has been shown to increase the stability of internal fixation, maintain the height of the humeral head to prevent varus, and reduce the risk of screw penetration. Biomechanical studies have also shown that the medial column has an outstanding performance in increasing the stiffness, torsion resistance, and shear resistance of the locking plate. Although it has many benefits, there is no unified definition of its concept and specific region, and the existing classification does not include the medial column, therefore more researches are required to provide supporting information. The methods of medial column reconstruction mainly include locking plate combined with talus screw, locking plate combined with bone grafting, internal and external double plate combined support, locking plate combined with bone cement, and humeral cage. These methods have their own characteristics, however they will increase the cost of surgery and bring new complications. How to determine the best way of reconstruction is one of the focuses of future research. In this review, the concept of the proximal medial humerus column, the role of maintaining internal fixation, the role of biomechanics and the reconstruction methods are reviewed.

OBJECTIVE@#To compare the clinical effect differences between "'s five-needle method" and routine acupoint selection on allergic rhinitis and asthma syndrome.@*METHODS@#A total of 210 patients with allergic rhinitis and asthma syndrome were randomly divided into an observation group (105 cases, 4 cases dropped off) and a control group (105 cases, 4 cases dropped off). The patients in the observation group were treated with "'s five-needling method", and the acupoints of Feishu (BL 13), Dazhui (GV 14), Fengmen (BL 12), Yintang (GV 29), Shangyingxiang (EX-HN 8) and Hegu (LI 4), etc. were selected; the patients in the control group was treated with routine acupuncture, and the acupoints of Feishu (BL 13), Zhongfu (LU 1), Taiyuan (LU 9), Dingchuan (EX-B 1), Danzhong (CV 17), Yintang (GV 29), Fengmen (BL 12) and Zusanli (ST 36), etc. were selected. The treatment in the two groups was given once a day, 6 times a week, for 4 weeks. The score of symptoms and signs was observed before and after treatment as well as 1 month, 2 months and 3 months after treatment. The forced expiratory volume in 1 second (FEV1), peak expiratory flow (PEF) and eosinophils in peripheral blood were measured before and after treatment in the two groups. After treatment, the clinical therapeutic effect was compared between the two groups.@*RESULTS@#The total effective rate was 98.0% (99/101) in the observation group, which was superior to 94.1% (95/101) in the control group (0.05), and the total score of symptoms and signs in the third month of follow-up in the control group was significantly increased (<0.05). After treatment, FEV1 and PEF in the two groups were increased (<0.01), eosinophil count in peripheral blood was decreased (<0.01), and the improvement in the observation group was greater than that in the control group (<0.01, <0.05).@*CONCLUSION@#"'s five-needle method" can improve the clinical symptoms and pulmonary function, reduce the count of eosinophils in peripheral blood in patients with allergic rhinitis and asthma syndrome, and the curative effect is better than routine acupuncture.

Osteoporosis (OP)-related vertebral fractures often occur in the transitional thoracolumbar region, which are the earliest signs of OP deterioration in elderly patients. Such fractures can be regarded as a physical incident that occurs when the vertebral stress load exceeds the mechanical structural strength. To OP patients, preventing the occurrence of OP fracture is the primary goal for preventative measures, treatment and research of OP. This paper reviews the biomechanical research progress of osteoporotic vertebra compressive fracture (OVCF), and advocates early prevention of OVCF in OP patients, who should avoid activities that might exceed the security scope of the vertebral body load in daily life, so as to prevent subsequent fractures of the vertebral fracture cascade.

The spinal manipulation in traditional Chinese medicine (TCM) has a prominent clinical advantage in the treatment of chronic lumbar pain, such as low back pain, but the insufficient basic research on manipulation is a serious obstacle to its development. Using finite element analysis (FEA) can preferably simulate the mechanics under various kinds of spinal manipulation, analyze its action mechanism, test the hypothesis, standardize the operating practices, make quantitative, qualitative and optimal treatment plans, so as to provide an effective method for the basic research on manipulation therapy. This paper reviews the FEA studies of spinal manipulation in recent years, discusses the influence of different spinal manipulation on intervertebral disc, lumbar accessory structures, spinal loads and mechanical stability of the vertebral body. The results show that current FEA studies on spinal manipulation need to make the simulation method standardized and precise. Meanwhile, the research ideas of finite element method should be developed to guide the clinical application of spinal manipulation.

ABSTRACT

Objective To design a new correction device for elbow deformity with active orthopedic force based on Ilizarov technique, and discuss its curative effect on elbow orthotics in clinic. Methods A total of 44 patients with elbow joint dysfunction were randomly divided into treatment group (n=22) and control group (n=22), and treated with new deformity orthotic device and continuous positive motion, respectively. The functional recovery of the elbow joint for patients in two groups was evaluated after 3-month treatment. Results The range of motion (ROM) of elbow flexion-extension in treatment group was significantly larger than that in control group, and the effective rate for treatment group and control group was 90.91% and 81.82%, respectively. Conclusions The use of such correction device for elbow deformity showed good curative effect for treating elbow joint dysfunction, which is worth popularization and application in clinic.

ABSTRACT

<p><b>OBJECTIVE</b>To establish a simple, rapid, inexpensive and sensitive method for detecting hot region for mutations in exon 7 of PAH gene.</p><p><b>METHODS</b>High-resolution melting (HRM) technology was used to detect a c.728G>A mutation in exon 7 in 88 patients with classical type phenylketonuria. Suspected mutations were validated by direct DNA sequencing.</p><p><b>RESULTS</b>The results detected by HRM are in good agreement with the results obtained by direct sequencing.</p><p><b>CONCLUSION</b>HRM analysis is a simple, rapid, inexpensive and sensitive method for detecting hot mutational region in exon 7 of PAH gene.</p>

ABSTRACT

ObjectiveTo compare the efficacy of single-stage and two-stage revision prosthesis-relative chronic infection causing by methicillin-resistant Staphylococcus aureus(MRSA) after total knee arthroplasty(TKA) in rabbits, and evaluate the clinical feasibility of single-stage revision TKA. MethodsA new kind of prosthesis was implanted into the right knee joints of 48 New Zealand white rabbits following proper anesthesia. After 4 weeks, the dose of 5×105 colony forming unit MRSA was inoculated into every knee to establish prosthesis joint infection model. The rabbits were divided into 2 groups randomly: experimental and control group. Four weeks after inoculation, the treatments of the experimental and control group were singlestage and two-stage revision respectively. The levels of serum C-reaction protein (CRP) and Erythrocyte sedimentation rate (ESR) were monitored in ten phase, i.e. prior to primary arthroplasty and revision, at 1, 3, 5, 7days, and 2, 4, 6, 12 weeks after revision. Twelve weeks after revision, animals were sacrificed and joint samples were collected for bacterial culture. The positive results were judged as reinfection, and the negative results were judged as successful healing. ResultsFive rabbits were excluded out of the group for some reasons. The recurrence rates of infection in the experimental group and control group were 22.7% (5/22) and 14.3%(3/21) respectively after revision. The difference between them was statistically insignificant(χ2=0.102,P=0.750). The levels of serum CRP of the two groups raised, and reached their peaks at 3 days, then dropped into the normal level prior to primary arthroplasty at 4 weeks after revision. The difference between them was statistically insignificant (F=0.157, P=0.694). The ESR levels of the two groups elevated after revision, and reached their peaks at 5 days, then declined slowly into the original level prior to primary arthroplasty at 12 weeks. The difference between them was statistically insignificant (F=0.936,P=0.339). ConclusionThough the prosthesis-relative chronic infection caused by high virulence organism after TKA, the short-term efficacy of single-stage revision is similar to that of two-stage if the stain of pathogenic bacteria and its spectrum are obtained.

ABSTRACT

<p><b>OBJECTIVE</b>To study the mutations in exons 3, 6, 7, 11 and 12 of the phenylalanine hydroxylase gene (PAH) in Shanxi population.</p><p><b>METHODS</b>The mutations in exons 3, 6, 7, 11 and 12 and flanking sequences of PAH gene were detected by PCR-DNA sequencing, in 59 patients with phynelketonuria(PKU) and 100 healthy children from Shanxi province.</p><p><b>RESULTS</b>By sequence analysis, three single nucleotide polymorphism (SNP) Q232Q (CAA>CAG), V245V (GTG>GTA) and L385L (CTG>CTC) were detected in both the patients and healthy children, with the frequencies of nt 696, 735 and 1155 of the PAH cDNA up to 96.2%, 76.1% and 7.6% in patients respectively, and 97.0%, 77.3% and 8.3% respectively in the healthy controls. In addition, 72 different mutations accounting for 61.0% of mutant alleles were identified in the patients only. In exon 3, R111X, H64>TfsX9 and S70 del were found accounting for 5.1%, 0.8% and 0.8%; EX6-96A>G in exon 6 was found accounting for 10.2%. In exon 7, R243Q was the highest incidence accounting for 12.7%, followed by Ivs7+2 T>A(5.1%) and T278I(2.5%); the lowest incidences were G247V, R252Q, L255S, R261Q and E280K accounting for 0.8 %, respectively. In exon 11, Y356X (5.9%) and V399V (5.1%) were found; in exon 12, R413P and A434D were found accounting for 5.9% and 2.5%. In total, 9 missense mutations, 3 splice site mutations, 2 nonsense mutations and 2 deletions were included in 16 kinds of different mutations.</p><p><b>CONCLUSION</b>The mutation characteristics and distribution in exons 3, 6, 7, 11 and 12 of the PAH gene have been identified, and it suggested that the EX6-96A>G and R243Q were the hot spots of PAH gene mutations in Shanxi PKU population.</p>

ABSTRACT

<p><b>BACKGROUND</b>The aim of the current study was to investigate the role of interleukin (IL)-2, interferon (IFN)-gamma, IL-4 and IL-10 in a concordant hamster-to-rat cardiac xenotransplantation model.</p><p><b>METHODS</b>A hamster-to-rat cardiac transplantation was performed using SD rats as recipients of Golden Syrian hamster hearts. A total of 60 SD rats were divided into four groups and treated as follows: control group (n = 15); splenectomy group (n = 15); CsA group (n = 15); CsA + splenectomy group (n = 15). Levels of IL-2, IFN-gamma, IL-4 and IL-10 were measured by enzyme linked immunosorbent assay (ELISA). Sera were harvested at different time points in each group: day 1, and 3 as well as the day the xenograft stopped beating in the control group and CsA group; day 1, 3, 7, 14 and 30 in the splenectomy group and CsA + splenectomy group. The expression of P-selectin and intercellular adhesion molecule-1 (ICAM-1) was examined by immunohistochemical analysis of the xenograft after cardiac xenotransplantation.</p><p><b>RESULTS</b>Serum levels of IL-2 and IFN-gamma were upregulated in untreated (day 3) and splenectomy-treated animals (day 7) compared to CsA + splenectomy treated animals (day 7). IL-10 was upregulated in long-term survival recipients following splenectomy + CsA. Neither P-selectin nor ICAM-1 expression was detected in long-term survival xenografts.</p><p><b>CONCLUSIONS</b>Serum IL-2 and IFN-gamma were elevated following acute vascular rejection. Serum IL-10 was correlated to immunosuppression and protective effects in long-term survival rats following concordant cardiac xenotransplantation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the relationship between hepatic lipase activity and fatty degeneration of hepatocytes and explore the effects of tea polyphennols (TP) on the changes of hepatic lipase (HL) activity in rabbits with fatty liver.</p><p><b>METHODS</b>According to serum cholesterol and triglyceride (TC) levels, 19 rabbits were divided into fatty liver group (FL, n=6) fed with high cholesterol diet, TP group (n=7) fed with high cholesterol diet and 20mug/g/d tea polyphennols everyday orally, control group (n=6) fed with normal diet. After 8 weeks, the levels of serum TC, HL activity, HL activity and malondildehyde (MDA) in hepatic tissue were detected, and the pathomorphology of hepatic tissue were determined in all rabbits.</p><p><b>RESULTS</b>The fatty degeneration of hepatocyts in FL group was more severe than that in TP and control group. The serum TC level in TP group (16.87 6.58) mmol/L was higher than that (1.11 0.82) mmol/L in control group (t=5.786, p<0.05), but lower than that (28.49 5.99) mmol/L in FL group (t=3.968, p<0.05). The serum low density lipoprotein-cholesterol level in Tp group (5.10 4.19) mmol/L also higher than that (0.71 1.14) mmol/L in control group (t=3.763, p<0.05), but lower than that (12.15 1.95) mmol/L in FL group (t=2.478, p<0.05). The number of positive dots presenting HL activity level in 100 square micron, hepatic tissue in TP group (3.24 0.17) was higher than that (1.76 0.10) in FL group (t=-3.153, p<0.05), but lower than that (4.14 0.05) in control group (t=-2.902, p<0.05). The levels of MDA in hepatic tissue in TP group (44.66 26.18) nmol/mg was significantly lower than that (75.58 29.88) nmol/mg in FL group (t=2.261, p<0.05), but no evidently different from that (43.64 16.95) nmol/mg in control group. The plasma HL activity was no difference among the three groups.</p><p><b>CONCLUSION</b>The HL activity in hepatic tissue with fatty degeneration of hepatocytes was lower than that in normal liver. Tea polyphennols can increase HL activity in hepatic tissue and protect hepatocytes from fatty degeneration.</p>
